• You turn 65 this month. • You will turn 65 within 3 months from now. • You turned 65 no more than 3 months ago.
Relocation
• New Address: If you move to a new address that isn't in your current plan's service area or offers new plan options. • Returning to the US: If you move back to the US after living abroad. • Facility Changes: If you move into or out of a nursing home or rehabilitation hospital.
Loss of Coverage
• Medicaid: If you lose eligibility for Medicaid. • Employer or Union Coverage: If you leave coverage from your employer or union, including COBRA. • Other Drug Coverage: If you involuntarily lose other drug coverage that is as good as Medicare drug coverage.
Other Coverage Opportunities
• Employer or Union: If you have a chance to join other coverage offered by your employer or union. • TRICARE or VA: If you enroll in other drug coverage like TRICARE or VA coverage.
Involuntary Plan Changes
